What is a Scar?

A scar is a mark left on the skin after a wound or injury has healed. When the skin is injured, the body initiates a complex healing process to repair the damage. This process involves the formation of new collagen fibres, a protein that helps to strengthen and rebuild the tissue. Unlike normal skin tissue, the collagen in scars is arranged differently, making the scar tissue fibrotic, less elastic, and often less functional than pre-injury tissue. Furthermore, adhesions may form in the presence of excess or prolonged inflammation, both at the scar site on the surface and in the surrounding tissue below. Adhesion formation can restrict the normal smooth fascial slide and glide of the soft tissue ‘layers’ as they adhere to each other.

There are different types of scars, including:

- Hypertrophic scars: These are raised scars that remain within the boundaries of the original wound.
- Keloid scars: These extend beyond the original injury site and can become large and uncomfortable.
- Atrophic scars: These are sunken scars that result from the loss of underlying tissue, often seen in conditions like acne or chickenpox.
- Contracture scars: These occur when the skin "shrinks" as it heals, which can restrict movement, commonly seen in burn victims.

The appearance and characteristics of a scar depend on multiple factors such as the depth and size of the wound, its location on the body, the age and genetic predisposition of the individual, and how well the wound was cared for during the healing process. Scars may cause physical discomfort, such as itching, pain, and sensitivity, and can also lead to psychological distress due to their appearance. Various treatments, including scar massage, topical applications, laser therapy, and surgical options, can help improve the appearance and flexibility of scar tissue, alleviating both physical and emotional impacts.

When to Start

Scar therapy can be started from 8 weeks post-injury or surgery, as long as the scar is healing well and with the approval of a GP or consultant. The first session will be 60 minutes to cover a full consultation and treatment. Subsequent sessions would be 30 minutes for immature scars but fairly frequent to support the body as it lays down collagen. The therapy is also effective for older scars, making it beneficial at any stage of recovery; these sessions can be longer and less frequent.
